The enrolment system aims to provide financial assistance to patients with kidney disease to cover their expenses of the costly medicine (EPOIETIN) and laboratory procedures while undergoing dialysis. Under the enrolment system, patients with renal disease may avail of the following PCSO assistance:

  1. PhilHealth Members
    Hemodialysis Peritoneal Dialysis*
    • A maximum annual assistance of Php84,000.00 for medicines (epoietin) at Php14,000.00 per claim for a total of 6 claims per year.
    • For those who avail the Z package, a maximum annual assistance of Php84,000.00 for medicines (epoietin) at Php14,000.00 per claim for a total of 6 claims per year;
    • For those who are under the case rate, a maximum of annual assistance of Php120,000.00 for dialysis solutions at Php20,000.00 per claim for a total of 6 claims per year
    • *A certification of the type of Philhealth benefit package under peritoneal dialysis must be presented to determine the appropriate form of assistance

    Assistance may be processed and released in six (6) equal tranches one after the other at any time upon presentation of certification of ongoing treatment from the attending physician.

  2. Non-PhilHealth Members
    Hemodialysis Peritoneal Dialysis
    • A maximum annual assistance of Php84,000.00 for medicines (epoietin) at Php14,000.00 per claim for a total of 6 claims per year.
    • A maximum assistance of Php20,000.00 for procedure every other month
    • A maximum assistance of Php35,000.00 for dialysis solutions every other month

    Non-Philhealth members are required to submit a certification of PHIC non-membership from the attendant dialysis center.

Documentary requirements for inclusion in the Enrolment System for Hemodialysis and Peritoneal Dialysis Patients include the following:

  1. Upon Enrollment / First Claim
    Upon enrollment the patient is required to appear personally and must submit the following:

    • Duly accomplished Application Form;
    • Original Medical Abstract duly signed by the attending physician/Nephrologist with license number;
    • Prescriptions
      • Philhealth members
        1. Hemodialysis & Peritoneal Dialysis Z – medicines (EPO)
        2. Peritoneal Dialysis case rate – PD solutions
      • Non-Philhealth members
        1. Peritoneal Dialysis – PD solutions
    • Official quotation from the dialysis centers/hospitals;
    • Updated laboratory results;
    • Certification of acceptance of Guarantee Letter (GL) from the dialysis center/hospital;
    • Certification of PhilHealth membership status including type of package for Peritoneal Dialysis (Z or case rate);
    • Valid ID (patient and designated representative if there is any for the succeeding releases)
  2. Succeeding Claims
    On succeeding claims of PCSO assistance, patient may assign or designate a representative. The index card/client-social worker agreement issued upon enrollment must be presented to claim the Guarantee Letter (GL)*.

    • Certification of ongoing treatment including the last date of treatment from the Nephrologist or attending physician;
