ENDOWMENT FUND PROGRAM (EFP)


This program allocates funding assistance to government and private hospitals assistance to indigent patients in need of medical care, medicines, drugs, medical and surgical supplies, and diagnostic procedures for the management and treatment of various illnesses.

OBJECTIVES

  • General : To grant fund to hospitals and other health facilities making health or medical services accessible to PCSO-authorized indigent patients
  • Specific : Enable charity patients access to medicines, drugs, medical supplies, implants, blood, diagnostic and laboratory procedures

CRITERIA

  • Devolved Government Hospitals - more than 50% Occupancy Rate
  • DOH Hospitals, Private and Other Health Agencies-more than 80% Occupancy Rate
  • Capability to provide medical examination and or intervention
  • Has employed Social Worker

COVERAGE

Maximum assistance to PCSO-authorized charity patients of P50,000.00 /yr for:

  • Medicines, special drugs (chemo, dialysis, facts 8,9 IGIV, etc)
  • Consumables (reagents, supplies, dye)
  • Diagnostic procedures (MRI, 2D Echo, etc)
  • Metallic shunts, implants
  • Equipment Rental (life-saving ventilator, respirator, etc)
  • Hearing aid, prosthetic devices
  • Brachy therapy, radiotherapy

Additional assistance may be requested on a case-to-case basis as justified by Social Worker to Approving Authority.

APPLICATION

The requesting Hospital/health facility shall write a letter to the Chairman/General Manager and submit the following documentation requirements:

  • Project Proposal
  • Hospital/Agency Profile
  • Certification of Availability of Referral Facilities
  • DOH License to Operate/Accreditation Certificate
  • Latest Accomplishment Report

Additional assistance may be requested on a case-to-case basis as justified by Social Worker to Approving Authority.

  • SEC Registration with Articles of Incorporation and By-Laws
  • Financial Statement for Two (2) Years
  • Agency vision, mission, goals and programs and services
