REQUIREMENTS FOR FINANCIAL ASSISTANCE PROGRAMS FOR THE PURCHASE OF MEDICINES

  1. Project Proposal approved/signed by its officers, must include the following information:
    • Project Rationale
    • Project Objectives
    • Area Coverage
    • Target Clientele
    • List of Medicines (for purchase of medicines)
    • Economic and Geographical Profile of the Area
    • Program Plan and Implementation
    • Budgetary Requirements of the Project
    • Monitoring and Evaluation

  2. Endorsement Letter from the Regional/Provincial/City/Municipal Health Office (for purchase of medicines)

  3. Price Quotation from at least three (3) suppliers. Quotations should strictly indicate name, signature and designation of the issuing officer, telephone number and address which should be written on their official stationery. (For purchase of medicines)

  4. Additional Requirements for NGO's:
    • Certificate of Registration with Securities and Exchange Commission (SEC) and/or either the Cooperative Development Authority (CDA) or the Department of Labor and Employment (DOLE)
    • Authenticated copy of the latest Articles of Incorporation or Articles of Cooperation
    • Financial Reports audited by an independent Certified Public Accountant for the past three years preceding the date of the project implementation
    • Disclosure by the NGO/PO of other related business, if any and extent of ownership therein
    • Work and Financial Plan (WFP), and Sources and Details of Proponents Equity Participation in the project
    • List and/or photographs of similar projects previously completed by the NGO/PO, if any indicating the source of fund for implementation
    • A sworn affidavit of the Secretary of the NGO/PO that none of its incorporators, organizers, directors or officials is an agent of or related by consanguinity of affinity up to fourth civil degree to the officials of the GO authorized to process and/or approve the proposal, the Memorandum of Agreement and the release of fund
    • Set of Officers of the NGO/ Beneficiary – Organization
