PROGRAM DESCRIPTION

The Institutional Partnership Program is one of the interventions of the Philippine Charity Sweepstakes Office for the various government and non-government organizations (GOs and NGOs). It aims to augment the resources of the various welfare agencies and charitable medical facilities in order to further extend the charity services of the government, particularly in the medical and health needs of the poor and disadvantaged sector in the local communities. PCSO-NGO partnership can be described as a relationship rooted in the acceptance of both parties of their shared vision and responsibility for the delivery of social services within policy and legislative frameworks governing a country’s response to its social needs and problems.

It is an acknowledgement, acceptance and respect by each party of the other’s distinct, but mutually complementary and interdependent roles for the attainment of shared goals.

OBJECTIVES

The PCSO Institutional Charity Assistance Program shall endeavor to specifically accomplish the following:
1. Provide indigent patients with free access to necessary health, medical intervention.
2. Extend charity services to the poor and disadvantaged sectors of the society.
3. Facilitate speedy settlement with partner agencies of provided services to needy clients.

APPLICATION REQUIREMENTS

1. Who are eligible?

a) Residential Welfare Facilities
• RF for the mentally challenged/special children and elderly, and those requiring special care
• RF for the infants and juvenile

b) Medical-Related Organizations (MRO)
MRO undertaking surgical operations:
– Pterygium
– Cataract
– Harelip and Cleft Palate

c) Welfare Organizations
– WF undertaking nutrition/feeding program

d) MRO providing prosthetics:
– Below Knee
– Above Knee

e) MRO providing blood services
2. What are the documentary requirements?

a) Letter Request to be addressed to the Chairperson or General Manager of PCSO
b) Completed Application Form
c) Project Proposal
d) Additional documents:
• Endorsement Letter from the DSWD
• Photocopy of Certificate of Registration or License from the DSWD
• SEC Registration and By-Laws
• Set of Officers
• Audited Financial Report for the previous year
• Disclosure of other related business and extent of ownership
• Work and Financial Plan and Sources and Details of Equity Participation in the Project
• List and/or photographs of similar projects previously completed with sources of funds.
• Sworn Affidavit that none of its incorporators, directors or officials is an agent of or related by consanguinity or affinity to the PCSO officers authorized to process and/or approve the proposal, the MOA and the release of funds.
