  1. Apply through an online form which is available here . Upon submission, your application will automatically be send to Kaibigan Ng PCSO Office. Our staff will process your application.


  2. Fill out an application form in MS Word format which is available here.  Print the MS Word form using a short (8 1/2" x 11") bond paper, then submit the fully accomplished form to:
