BRIEF DESCRIPTION OF THE PROGRAM

Provides fund augmentation to upgrade government hospitals and other health institutions, thus facilitating efficient and effective medical diagnosis and treatment.

OBJECTIVE

To provide financial assistance to medical facilities (government or private) and other health institutions for the purchase of medical equipment with the end-view of rendering accessible and quality/health services to indigent patients.

  1. Hospital Profile which includes the following:
    • Authorized bed capacity
    • Bed occupancy rate
    • Alternate source of funding
    • Case mix
    • Manpower complement to use/operate the requested equipment   Provision/availability of an appropriate space for the equipment to be functional (electric load, sufficient water supply if needed)
    • Commitment of the hospital to fund an equipment maintenance program to sustain the operations/functionality of the requested equipment including future purchase of necessary reagents/supplies if applicable
    • Budgetary allocation for operation (cost per patient per day)
    • Statistical report
    • Cost Benefit Analysis
  1. Price quotations from three (3) suppliers of medical equipment with signature over printed name of the official representative of the dealer written on their official stationary
  1. Certification/Recommendation from the Department of Health for the upgrading of hospital facilities (purchase of medical equipment)
  1. Additional Requirements for NGOs
    • SEC Registration with Articles of Incorporation and By-Laws
    • Financial Statement for the past two (2) years/Financial Plan
    • Track Record or Accomplishment Report for the past two (2) years
    • License to Operate or Accreditation from any appropriate agency
