Documentary Requirements

1. Request from LGUs (Provinces/Cities/Municipalities)
   1. Letter request signed by the Local Chief Executive (LCE) addressed to the General Manager
   2. Certificate of Availability of Funds for maintenance and other operating expenses (MOOE)
   3. Sangguniang Bayan/Panlungsod/Panlalawigan Resolution authorizing the Local Chief Executive to sign the DOD and receive the MTV
   4. Valid government-issued ID of the LCE
2. Request from DOH/LGU-run hospital and other government agencies
   1. Letter request signed by the head of the hospital/agency addressed to the General Manager
   2. Certificate of Availability of Funds for maintenance and other operating expenses (MOOE)
   3. Official appointment and/or designation of the head of the agency authorizing him/her to sign the DOD and to receive the MTV
   4. Valid government-issued ID of the head of the hospital/agency
